Expert Witness

I have acted as an expert witness in several cases where fatalities have occurred. For example, in the Ladbroke Grove rail collision, I acted for Network Rail, using Human Factors methods, to explain why it took 20 seconds for the signalman to intervene following the Signal Passed at Danger (SPAD) by the train. Other examples include analysis of a fire onboard an oil rig, road collision involving an automated vehicle, and a crash of a passenger airline into the ocean. The expert witness reports explain not just what happened but also the contributory Human Factors that were involved and why they led to the incident.
