I have used Human Factors methods to investigate collisions, accidents and incidents in many different domains (air, maritime, road, rail, energy distribution, oil and gas, healthcare, etc.). These analyses have shown the power of Human Factors methods to uncover deeper, systemic, causes often neglected by other approaches.
